Parish Registration Process

In order that our parish can better serve your needs, we ask that all parishioners register with the parish. This can be accomplished by filling out a parish registration form ( see below ) and submitting to the office in one of several ways:

• Bring it to the parish office. During business hours, turn it in to a member of the parish staff. Outside of business hours, deliver it via the mail slot in the parish office door off of the main hallway of the Family Life Center.
• Bring the completed form to mass and put it into the collection basket.
• Mail the completed form to the parish office at this address .
• Fax the completed form to the parish fax number .
